How cautious do I need to be when buying a fan/heatsink assembly on eBay?

VERY cautious. If it was RAM or a video card you should be cautious, but it would be possible. But used heat sinks just won't work. period. I would recommend finding a well-rated trader and check out the details thoroughly.
